Interesting topic, Bill. Buffalo Wings, nachos, fajitas, Cesar and waldorf salads all started as regional dishes and are now everywhere. I think dishes become national when people travel and take them places and when the dishes contain relatively simple ingredients that can be replicated relatively easily . And sometimes (ulp) chains introduce an inferior version of the original to a bigger audience, some of whom go on to seek higher quality versions.
